Alongside our Primary Care Network (PCN) Clinical Pharmacists, our practice ran a very successful pilot project alongside the local hospital and their heart failure team. The aim of the project was to optimise the treatment and care of patients who were under their service but close to home. Our specially trained independent prescribing pharmacists, working with specialist heart failure nurses, worked collaboratively leading to better treatment regimes for those with heart failure. This important piece of work ensured that patients had better outcomes as well as a better quality of life.
At the surgery, we employ a multidisciplinary team of healthcare professionals, not just doctors, and their mixed skillsets allow for us to deliver these important services at neighbourhood level, without referring patients into the hospital setting and adding to outpatient waiting lists.
